Privacy policy
How contact enquiries and consent records are collected, shared and stored, with access, correction, complaint and unconfirmed privacy details.
$99 doctor consult
$99 doctor consult — the fee for a consultation with a registered doctor only. Any treatment, tests or medicines are separate and decided by the doctor.
Who is responsible for this policy?
This policy describes the enquiry website’s current approach using Australian Privacy Principles-style headings. It is not a claim that every legal or operational detail has been confirmed. The legal entity, ABN, contact details and the disclosures marked below remain placeholders and require completion before launch. Weight Doctor Match is an information and enquiry website, not a clinic.
Privacy contact: [contact email: placeholder]
What information do we collect?
The enquiry form collects your first name, mobile number, email address, state and postcode, confirmation that you are 18 or older, a non-clinical enquiry topic, preferred contact time if supplied and consent to share the enquiry. State and postcode appear as one combined field. The consent box starts unticked and must be selected if you choose to submit.
We also record the consent version, consent time, submission time, source site, service category, internal destination tag and landing page path. The current consent version is share-v1. The landing page is recorded as a path, not its query or fragment. We do not ask for weight, measurements, health history, symptoms, photos or a Medicare number. Do not put health information in a contact field. The reflection points on this site are display only and are not stored.
Why do we collect these details?
We collect contact and enquiry details to pass your request to a referral partner so a registered doctor’s team can contact you about an assessment. Consent records show which sharing notice applied when you submitted. Source and routing details help identify which site and enquiry pathway the request came through. We do not use this form to diagnose you, make a clinical decision or enrol you for marketing.
Submitting is your choice. If you do not agree to sharing, do not tick the consent box; the form will not submit without it. You can still read the information and speak to your own GP or another provider. A preferred contact time is a preference, not a confirmed appointment or response timeframe.
Who receives my information?
With your consent, your enquiry details are passed to our referral partner clinic. A service connected to that clinic may receive information only if you later choose, as explained in the collection notice. Ask the clinic for its own privacy information before sharing further details during care. This website’s policy does not replace the clinic’s policy for clinical records.
Enquiries currently use an interim referral pathway. The receiving referral partner’s identity, responsibilities and relationship to the clinic must be confirmed in the placeholder disclosure. Routing is not clinical triage. Authorised site enquiry handlers may access details to manage the request, and a notification containing enquiry details may be sent to designated site enquiry contacts when email sending is configured. We do not sell your information.
[Interim referral partner: placeholder]
We currently pass enquiries to one referral partner clinic. We are paid/related to that partner as follows: [disclosure: placeholder].
How are details stored and processed?
Submitted enquiries are stored privately and are not available for public viewing or public editing. Website hosting, enquiry storage and notification providers may process contact details to operate the service. These technical providers do not become a clinical service by handling an enquiry. The final provider list, access arrangements and locations need to be verified before launch.
Whether any information is stored or processed overseas, the countries involved and relevant safeguards have not yet been confirmed. The disclosure remains a placeholder rather than an assurance that processing occurs only in Australia. Do not treat this incomplete statement as a confirmed account of every provider’s practices. Those details must be supplied in the completed policy.
Overseas storage and processing: placeholder. Provider locations, countries and applicable safeguards must be confirmed before launch.
How long do we retain an enquiry?
The retention period and deletion arrangements have not yet been confirmed. A completed policy needs to explain how long contact enquiries and consent records are kept, why that period is needed and how removal from active systems and backups is managed. We do not invent a retention period or say that submitting a request causes immediate deletion.
Once a partner has received an enquiry, its own retention obligations and privacy practices may apply to the copy it holds. Ask that partner about its arrangements. A request to this website does not by itself remove records held by another organisation. The unconfirmed retention information is marked below.
Retention period: placeholder. The period and deletion arrangements must be confirmed before launch.
How can I access, correct or withdraw an enquiry?
You can request access to or correction of the enquiry details held by this website through the privacy contact once that channel is confirmed. Explain the request using contact information only; do not include clinical history. Reasonable identity checks may be needed before details can be provided or changed. The access process and any applicable requirements must be confirmed in the final policy.
You can also ask for further sharing or contact to stop. A request cannot undo a disclosure already made to the partner, and you may need to contact the partner separately about its copy. The privacy contact remains a placeholder, not a functioning email address. There is no invented contact channel on this page.

How do I make a privacy complaint?
Raise a concern with the website’s privacy contact once confirmed, describing the handling of your enquiry and the response you are seeking. The complaint handling steps and response arrangements require completion before launch. For a concern about the partner clinic’s handling of information, ask the clinic about its own privacy complaint process.
The Office of the Australian Information Commissioner, or OAIC, provides information about privacy rights and making a complaint. Its website explains the steps to take and whether it can consider a concern. The official link below is information, not a promise about the handling or result of a complaint.
Do you use analytics or advertising trackers?
No analytics or advertising trackers are currently used on this website. Enquiry source and landing page records are part of handling the request, not a marketing tracking programme. We do not sell personal information. Any future change to collection, recipients or use needs to be reflected in the policy and relevant collection notice before it applies.
Read the notice above the submit button as well as this policy. The consent wording uses version share-v1. This site is not a clinic; we don’t prescribe or supply; the doctor decides.
